1. Fill this form completely and submit it
2. An EPV Screens sales rep will help follow up once the submitted info is received in
3. EPV Screens requires an Official Non-Cancelable Purchase Order from reseller/distributor
4. EPV Screens requires 100% payment upfront once the order is confirmed. For orders placed through an EPV Screens dealer, payment terms may apply.
5. Estimated time of delivery for custom screen is up to 150 business days from the date of PO/full payment
6. For the maximum projector screen sizes by series, please refer to the PDF file.
7. Please review the Quality Check Standards before submitting a customer screen request. If you do not agree with those standards, please do not submit the request or place order
